It’s time again to….Ready. Set. RISEUP.

Cutler Health Center is offering biometrics screenings as part of the University of Maine System’s new Ready. Set. RISEUP Program. UMS, in partnership with Provant and Cutler Health Center, will be providing biometric screenings to University of Maine employees, spouses and domestic partners that are enrolled in UMS Open Access Quality Incentive CIGNA Insurance plans. By completing Level 1 – which consists of a biometric screening (5pts), an online health assessment (5pts), and earning 10 additional wellness points – prior to March 31, 2014, you will be eligible to receive the reduced premium incentive*.

Your Biometric screening consists of:

  • Total Cholesterol (TC)
  • HDL (good cholesterol)
  • TC/HDL Ratio
  • Glucose (Fasting or Non Fasting Acceptable)
  • Blood Pressure
  • Height
  • Weight
  • Body Mass Index
  • The Cholesterol and Glucose screen is performed by a finger stick, where blood is drawn from the finger.  You should expect your entire screening to take 20 minutes.

    Please bring your CIGNA insurance card with you at the time of visit. You may access Cutler Health Center by utilizing the main entrance.  If you park in patient parking you will need to obtain a permit during check in.

    Please know these appointments fill quickly thus take advantage of an appointment at your earliest convenience. The RiseUp program Level 1 incentive deadline is March 31, 2014**. Cutler Health Center will be releasing additional biometric appointment dates throughout this timeframe.

    If you are not participating in a biometric screening on campus, and planning to obtain the biometric Screening results from either a Primary Care Physician (PCP) visit since July 1, 2013 or a currently scheduled visit with your PCP prior to March 31, 2014, those results can be mailed/faxed by your PCP directly to Provant. Biometric Screening Results Form to bring to your PCP and have the results sent into Provant.

    *Incentives are available to non-represented employees and union members who are participating in the Quality Incentive Plans (QIP).

    **Complete Level 1 by March 31, 2014, to earn your premium incentive at the earliest possible date. Final deadline for Level 1 completion is November 30, 2014. NOTE: Please remember that if you have an enrolled spouse/domestic partner that they too will need to complete the criteria for Level 1 in order to earn your premium incentive.

    Complete all steps and earn all points by November 30, 2014, to earn your cash reward.
