Programs - Sport Clubs - Purchasing Equipment Date Requested Sport Club Requester Phone In order to try to provide you with the easiest equipment purchase experience, please take a moment to fill out this request. Procedure Checklist: 1. Verify with the Sport Club Coordinator your club's available funds 2. Verify with the Sport Club Coordinator that the equipment you want to purchase is appropriate for your club 3. Select a company you want to purchase from (ie the Vendor) 4. If the Vendor is web-based, confirm they accept purchase orders 5. Obtain the following information: Name of Company Name of individual Address City State Zip Phone number (area code) FAX number (area code) Email address Web address Ask Vendor for a "Quote" only, not an invoice. DO NOT place an order for any equipment at this time. Remember to include shipping and handling costs. Note: The University does not pay tax!! 6.
