Immunization Deadline

We MUST receive your immunization records by January 12, 2009 or you WILL NOT be able to attend classes.

The Immunizations You Must Show Proof Of

  • One dose of Tetanus/Diphtheria given within the last ten years
  • Two doses of Measles, Mumps and Rubella (MMR) given after (not on) your first birthday and after December 31, 1956.


All students (defined as any person entering school who attends school full time or who is a candidate for a degree, diploma or graduate certificate AND all part time students who are degree candidates are included under this definition) must have documentation of two doses of MMR.

The Documents You Need to Send
We need a signed or stamped record of your immunizations from your high school, college, military base, or private health care provider.


Where You Need to Send Your Documents
Mail your official immunization records to:

Or fax your official immunization record to:

If you are unable to locate your records, first contact your family physician for vaccination administration and, if necessary, Cutler Health Center can provide the required immunizations. However, due to a change in federal funding for adult vaccines, there will be a cost associated with this service.
