How do I get started?

  1. Send documentation of disability
  2. Fill out and print our DSS Intake Form (PDF)
  3. Make an appointment to see one of our professional staff by calling (207) 581-2319; your appointment will be at 121 East Annex (click for directions) on the University of Maine campus
