MaineCard Application Form

Lost your MaineCard, or are you a new University of Maine student, faculty or staff member and would like to get a MaineCard? Print out this form and bring it to the Student Service Center on the first floor of the Memorial Union Building. Please bring a valid photo ID for identification verification.

Department MaineCard Contract

Departments at the University of Maine can apply for a Departmental MaineCard, and put funds on their card. Print out a copy of the contract, and send the completed form to the Department of Financial Services for approval.

Community Meal Plan Contract

Black Bear Dining realizes that commuter students are an important part of our campus community. We have Community Meal Plans for those off-campus students who wish to dine on campus. This can be a welcome change from preparing your own meals or as a convenience to dine on campus. Commuter students may choose from a Resident Meal Plan or a Community Meal Plan.

Commuter Dining Funds Contract

Commuter Dining Funds allows students to take advantage of the

computerized declining balance program and allows the flexibility of charging these amounts to the student account.

Non-Photo MaineCard ID Application

If you take classes remotely and are an undergraduate or graduate student at the University of Maine, you qualify for a non-photo ID.  Please fill out this form and send it in to us to process your request.  IDs for undergraduates will be sent to the Continuing Education division for distribution.  For graduate students the IDs are sent to the graduate school.
