  1. After filling in your name and Grower # above, skip to the bottom if you are looking for the harvest survey.
  2. Simply leave blank any box that is not applicable or that you don't feel like answering (unless required).
  3. To protect your identity, enter a Grower # of your own choosing, but use the same # each time you submit anything so I will know it is information that is truly coming from you.
  4. Certain box entries are required, such as 'Your Name' and 'Grower #.'
